Qualifications
  • At least 18 years old
  • High school diploma or GED
  • Current certification in CPR/First Aid
  • Driver Abstract
  • Current certification in CPR/First Aid
  • Valid driver’s license
  • Vehicle in good running order
  • Current auto insurance
  • Must be able to lift up to 75 lbs
  • BCI/BCII prior to providing services (must be clear of any misdemeanor or felony conviction of abuse and neglect of a child and/or abuse and neglect of an incapacitated adult). Plus other Registry checks
Pay Range:

  • $8.50 - $9.50 per hour
Responsibilities
  • Advocate for the client’s care, safety, and welfare as an active member of the treatment team of the person served.
  • Assist in the on-going development and implementation of each person’s training programs.
  • Document all activities through progress notes and data collection as outlined within the individual treatment plan.
  • Provide transportation as needed.
  • Attend and participate in regular staff training and scheduling meetings
  • Maintain confidentiality and privacy regarding all client’s information.
  • Respect the rights of the client and the client’s family.
  • Complete daily documentation of all agency required material.
  • Complete incident reports in accordance with policy and procedures.
  • Understand and implement the agency’s philosophy, policies and mission.
  • Understand and follow the agency chain of communication.
  • Work cooperatively with other team members to carry out the goals and objectives of the client.
  • Serve as a liaison between the agency and the community in daily activities with clients.
  • Ensure the safety and well-being of the client on a daily basis.
  • Develop and foster a professional relationship with the client based on mutual respect and trust.
  • Report any actual or suspected abuse/neglect in accordance with policy and procedure.
Training requirement

Training requirement prior to direct contact with consumers: (at least 8-hours yearly): .

  1. Overview of serving individuals with DD
  2. Consumer rights and confidentiality.
  3. Over view of basic principles and requirements of providing HC BS waiver services.
  4. The requirements of rule 5123:2-17-02 of Administrative Code relating to incidents adversely affecting health and safety (MUI &UI)
  5. Universal Precautions for infection control, including hand washing and the disposal of bodily waste.
